PHYSICIAN CONTINUING EDUCATION

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of CMEsolutions and Gaffney Events. The CMEsolutions is accredited by the ACCME to provide continuing medical education for physicians.


The CMEsolutions designates this live activity for a maximum of 13 AMA PRA Category 1 Credits.™  Physicians should claim only the credit commensurate with the extent of their participation in the activity.

COURSE EVALUATION & CREDIT REQUEST FORM

PERFUSIONISTS (CCP) CONTINUING EDUCATION

This activity has been submitted to the American Board of Cardiovascular Perfusion for approval to award Category I Continuing Education Units. The American Board will allot the following Category I CEUs to those perfusionists who attend the meeting as follows:


Saturday, October 29 - 10.5

Sunday, October 30 - 4.2 

Total Possible - 14.7

PARTICIPATION REQUIREMENTS TO CLAIM CCP CREDIT

  1. Attend/Participate in Sessions – credit given commensurate with attendance
  2. Scan badge each day
  3. Sign the CCP Sign-in Sheet for each day, located at the registration desk
  4. All participants who have signed-in will receive an email after the conference with details about how to obtain their CE certificate

The St. Louis Shock & Heart Failure Symposium will be supported through educational grants and exhibit fees from commercial supporters. The Course Directors will develop the educational content without any influence from commercial supporters. A complete list, along with appropriate acknowledgement will be given to all supporters at the time of the meeting. Exhibits will be available for viewing at all scheduled breaks.
